Event AV Checklist – Client Submission Form

Please complete and submit this form at least 1 MONTH prior to your event. This ensures
smooth coordination and execution of all audiovisual elements.

Power Requirements for External LED Panels or Equipment



If your DJ or decor company is bringing in LED panels or any large-scale lighting, you must specify how much additional power is required. ( Please feel free to talk to Power Audio Visual )
Standard wall outlets cannot support LED panels.

Failure to select power requirements in advance may result in technical complications or power failure during your event.

Final Notes

• All files must be submitted at least 4 days before the event
• All files should be tested 48 hours prior to ensure compatibility
• Our technician will follow your itinerary closely for cues
• Submit files via our Google Drive upload link or USB in person
• Payment for all services is due 5 Business Days Before Event
